| Functional Behavior Assessment
A functional behavior assessment (FBA) will be conducted
whenever a student's placement has been changed due to behavior.
The purpose of the FBA is to address the behavior impacting the
change in placement and to develop a plan to prevent the re-occurrence
of that behavior. The functional behavior plan does not have to
become a part of the IEP. It will need to
be modified regularly. If it is not a part of the IEP, adjustments
can be made without convening the IEP team. Parents should always
be included when adjustments in the behavior plan are made. If the
student has an existing behavior plan as part of his or her IEP,
the IEP team may convene to address the appropriateness of the plan.
An additional FBA and plan will not need to be done.
Procedure:

The school psychologist will schedule an IEP
review or a functional behavior assessment and will send prior
written notice, a meeting notice and procedural safeguards to
the parents.

The IEP team will conduct a functional
behavior assessment and develop a functional behavior plan.
The process in developing a functional behavioral assessment
is as follows:
- Identify the behavior that needs to change.
- Collect data on that behavior.
- Develop a hypothesis about the reason for the behavior.
- Develop an intervention to help change the behavior.
- Evaluate the effectiveness of the intervention.
Timeline
Either before or no later than 10 days after the disciplinary action.
